RFP: Tobacco Quitline and Data Collection Services for the Oregon Tobacco Prevention and Education P

				The State of Oregon, Oregon Health Authority (OHA), Center for Prevention and Health Promotion, Health Promotion and Chronic Disease Prevention Program, Tobacco Prevention and Education Program, "Agency," requests Proposals to provide tobacco cessation services, including Quitline and data collection and reporting services. 
 
The agency expects to award a single Contract not to exceed $2,000,000 for the period December 1, 2019 through November 30, 2021. If Agency determines that the Work performed has been satisfactory, Agency may, at its option, amend or extend the Contract for additional time and for additional dollars without further solicitation for a cumulative maximum total Contract term of up to 7 years for additional services reasonably within the scope of this solicitation, including increasing or decreasing service levels due to changes in Agency's authorized budget, and modification of services and service delivery to reflect best practices and emerging client service models. 
 
All persons or firms submitting Proposals are referred to as Proposers in this Request for Proposals (RFP); after execution of a Contract, the successful Proposer will be designated as Contractor. Additional details on the Scope of the goods or services or both are included in the Scope of Work section.
